- What services do we provide?

The Riviera Outreach service provides an alternative to moving into residential care, we offer long-term and live-in support if necessary to people with high dependency support needs who wish to remain living on their own in & around the Torbay area.

We promote real choice in practice between care at home & independent living and can help clients in several different areas.

These are some examples of how support may be offered:

Although each client's individualised package of care may identify tasks that are more specific to their requirements, we will regularly discuss the progress of clients support & any issues that need resolving.

We offer our domiciliary service to clients living in our supportive living accommodations Rydal Court & The Braddons, & we also, on request, provide the service in the local community & surrounding areas.

- staffing @ Riviera Outreach

We have a staff team who provide the service which comprises of a manager, team Co-ordinator and support workers.

Our support staff are drawn from our existing resources based within the Supportive Living schemes currently operated by Riviera Support Ltd (Rydal Court) & Riviera Support (Southwest) Ltd (The Braddons).

- How is the service is requested & delivered?

On receiving a completed referral form & risk assessment from a statutory service professional we will offer a no-cost initial client assessment visit where we can discuss needs, answer any question you may have about the company & leave you with our documentation (Brochures, terms & conditions and policies etc).

It will also be explained to you about costs that you may incur (if any).

We will ask the client (or their representative) to complete a care/support plan.

Once this has been accepted, we will confirm the client's move in date, support worker & the manager who will be responsible for their support.

Finally, we will require a signed copy of the contract and terms & conditions returned to us before the client's move in date & the start date of their support.

- Riviera Outreach Statement of Purpose & CSCi report

To ensure our Riviera Outreach service is beneficial to all participants (both the recipients & providers of the support); we have developed a Statement of Purpose covering the supportive accommodations' aims & objectives.

Riviera Outreach is registered with The Commission for Social Care Inspection (CSCI) who inspect the service we provide on a regular basis to monitor the social care services on offer, & they produce a report to show their findings which is a public document and available to anyone who wishes to see it.
